Required Immunizations

Required Immunizations must be updated before your student can attend classes. Visit Texas Health and Human Services for the required immunizations for school.

School Vaccination Requirements

Vaccine / Grade Level

Requirements

Diphtheria/Pertussis/Tetanus/Td DTP/DTaP/DT

4 years of age through 6th grade

Minimum of four doses of any combination of diphtheria-tetanus-pertussis containing vaccine will meet the requirement if the fourth dose was given on or after the fourth birthday.

Td

7 years and older

Three doses of a tetanus-diphtheria containing vaccine provided one dose was on or after the fourth birthday.

Tdap

7 grade

In addition to the above Td requirement, one dose for entry unless proof of tetanus/diphtheria containing vaccine within the last five years.

Tdap

8th - 12th grade

In addition to the above Td requirement, one dose required when ten years has passed since the last dose of tetanus/diphtheria containing vaccine.

Polio

All grade levels

For K-12th grade 4 doses of polio; 1 dose must be on or after the 4th birthday. However, 3 doses meet the requirement if the 3rd dose was on or after the 4th birthday. Not given after 18 years or older.

Measles, Mumps and Rubella

KG - 12th grade

Two doses are required, with the 1st dose received on or after the first birthday. Students vaccinated prior to 2009 with 2 doses of measles and one dose of rubella and mumps vaccine satisfies this requirement.

Hep B

All grade levels

Three doses required. For students' age 11-15 years, two doses of adult hepatitis B vaccine (Recombivax) are acceptable. Dosage and type of vaccine must be clearly documented (two 10 mcg/1.0 or Recombivax).

Hep A

KG - 11th grade

Two doses with the first one on or after the first birthday.

Varicella

All grade levels

Two doses with the first one on or after the first birthday.

Meningococcal

7th - 12th grade

One dose of quadrivalent meningococcal conjugate vaccine is required on or after the student's 11th birthday. Note: If a student received the vaccine at 10 years of age, this will satisfy the requirement.
