Observation Requests

Teacher Candidates,

If you would like to request placement in a Carrollton-Farmers Branch ISD school for your observation hours, please download the form in the folder below, fill it out, and submit it to the student teaching office. When you fill out all fields on the PDF form below and click on the “Submit” button, it will open your default e-mail program and attach the filled-out PDF to the e-mail.

In addition to the above form, we will also need to know how many observation hours you are required to perform on letterhead from your Alt Cert Program or College. We will process your request and seek a suitable placement for your observation hours. Furthermore, you are required to provide adequate Tuberculosis results from within the last year as well as submit to a criminal history check. You must submit a Criminal History Record Form to the Student Teaching office. Note: Observation requests usually take at least 10 business days to process once we receive your Tuberculosis test and criminal history record form. Some Requests cannot be honored. There is no guarantee we will find suitable observation assignment.
